Sugam Polyclinic provides medical consultation, appointment booking, and personalized diet planning services, accessible via WhatsApp and our online systems.
Your diet chart is prepared based entirely on the information you provide. We do not conduct an in-person physical examination as part of this specific service. The accuracy, safety, and suitability of your diet chart depend directly on the accuracy and completeness of the name, age, height, weight, health conditions, medications, allergies, and other details you submit through our form.
Your diet chart is prepared and reviewed by our doctor based on the information you provide, and is intended as general dietary guidance for the goals and conditions you have described. It is not a substitute for an in-person medical diagnosis, treatment plan, or ongoing clinical supervision.
If you have a medical condition, are taking medication, are pregnant or breastfeeding, or have any concern about whether a dietary change is safe for you, you must inform us accurately in the form and should consult the doctor directly before making significant changes to your diet.
Information and diet plans provided through our WhatsApp service, registration form, or dashboard are for general guidance and do not replace professional in-person medical diagnosis or treatment. Always consult the doctor directly for medical concerns requiring urgent or specialized attention.
Appointment bookings made via WhatsApp are confirmed automatically and assigned a unique reference number. We reserve the right to reschedule or cancel appointments when necessary, and will notify you accordingly.
A registration fee of ₹699 applies to the diet chart service, payable at the time of submitting the registration form. Diet plans are generated only after successful payment verification, except where waived at the doctor's discretion.
Because your diet chart is prepared and delivered digitally shortly after submission, the registration fee is non-refundable once payment is completed and your information has been submitted, except where required by applicable law.
You agree to provide accurate, complete, and truthful information when interacting with our WhatsApp service, registration form, or dashboard. You understand and accept that any inaccurate, incomplete, or misleading information you provide may directly affect the safety, accuracy, and appropriateness of the diet chart or other recommendations we provide, and that we are not responsible for outcomes arising from information you did not disclose or disclosed incorrectly.
We may update these Terms of Service from time to time. Continued use of our services after changes constitutes acceptance of the updated terms.
